Providers may order attendance verification forms by sending an email request to madisoncred@commerce.state.wi.us. the list
does not mean the class is currently being offered. To use
the search
feature, your computer must be set to allow pop-ups. After
choosing a
credential category, a list alphabetized by city will appear as a
